A patient with a large right nasal defect after excision of a skin cancer.   Most cancers in this area are due to sun exposure. A nasolabial flap is constructed.  The blue area is excised to allow the flap to be rotated and seated in place in a single stage.  The flap can also be brought over the intervening tissue, this will result in a slightly better cosmetic result, but requires a second procedure to cut the pedicle.

The flap is rotated in place and sewn into position with little tension.  The final result over 1 year later shows some fullness of the pedicle due to contraction during the healing process..
