Valsalva is the forceful expiration of air with the patient's mouth and nose held shut.  When performed in this fashion, air will be forced into the middle ear cavity and help treat eustachian tube dysfunction.  This patient has serous otitis media.  By forcing air in the middle ear, he is able to blow air into the middle ear which will help to resolve the middle ear fluid.
