Ear Maggots
: This is a very rare
condition. An insect

which lodged in the patient's ear and
laid eggs. In the ear can be seen the dead parent insect along with six
alive baby
maggots.
Valsalva
is the forceful expiration of air with the
patient's mouth and nose held shut. When performed
in this fashion, air will be forced into the
middle ear
cavity and help treat
eustachian tube
dysfunction. This patient has a large
posterior-superior
retraction pocket of the tympanic membrane. By
forcing air in the
middle ear,
he is able to blow out the pocket.
A Glomus Tympanicum is a paraganglioma of the
middle ear
cavity. It is a relatively rare,
highly vascular tumor. The tumor is locally invasive but can
rarely metastasize (spread by the blood stream or lymphatic system).
It can also be multi-focal with separate tumors developing in other
parts of the body.
The
eustachian tube
is a
passageway which extends from the back of the nose (nasopharynx) to the
middle ear. Its function is to let air into the
middle ear. It

The most common problem is that the tube is not letting enough air
in. This can lead to fluid in the ears (serous otitis media),
retraction pocket formation and
cholesteatoma
formation. This
patient has the opposite problem, her
eustachian tube
is open too much (patulous
eustachian tube). The video shows her eardrum retracting each time
she swallows. She has symptoms of popping, pressure and some
unsteadiness each time she swallows. This problem is usually associated
with extreme weight loss. One treatment for this condition is to
place an ear tube in the eardrum.
This patient is a 42 year old male who complained of having a whistling
out of his ear when he blew his nose. On examination he had a very
small pinhole perforation. The eardrum was mobile and he could
aerate his
middle ear
with
valsalva. When he did this, the eardrum
distended and a small perforation opened and a whistling could be heard as
air escaped from the
middle ear. This is one of the few cases where
an eardrum perforation was present and the eardrum was still mobile to changes in air pressure.
This is a very rare
condition which is usually caused by trauma. This case is in a 17
year old child who had a previous eardrum repair many years earlier.
The surgeon and the surgeon removed the
anterior
part of the ear canal so he could see the
anterior part of the eardrum.
He removed too much bone which resulted in the prolapse of the mandible's
(jaw bone) condyle into the ear canal.
