This is a very rare condition.  An insect which lodged in the patient's ear and laid eggs.  In the ear can be seen the dead parent insect along with alive baby maggots.

Maggots have long been used in ancient and modern medicine.  They only eat dead tissue, thus cleaning the wound and promoting healing.
